Case Managers provide outreach and support to seniors, individuals experiencing homelessness, and vulnerable community members throughout the Antelope Valley. This position focuses on helping clients access resources, apply for benefits, and maintain stability through consistent case coordination. Workers in this role should be compassionate, dependable, and able to balance administrative duties with direct community engagement. Strong communication and organization skills are essential to ensuring each client receives quality care and follow-up support.
We are hiring Case Managers / Client Support Coordinators to help connect clients with vital community programs and assist in ongoing support services. Applicants should be detail-oriented, empathetic, and able to work independently while managing multiple cases effectively.
Responsibilities
• Conduct outreach visits to homes, shelters, and community sites
• Provide one-on-one support for clients navigating resources and benefits
• Complete and maintain accurate client documentation
• Coordinate with the Johnson Family Support Services team for referrals and follow-up care
• Represent the organization professionally in community settings
Qualifications
• Experience working with elderly or homeless populations preferred
• Knowledge of local resources and social service systems
• Reliable transportation and valid driver’s license required
• Strong written, verbal, and interpersonal communication skills
